Genecradle Therapeutics' GC101 Meets Primary Endpoint in Phase III Clinical Trial for Spinal Muscular Atrophy

The Phase III confirmatory clinical trial of GC101 adeno-associated virus injection, independently developed by Genecradle Therapeutics, for the treatment of spinal muscular atrophy (5qSMA) has met its primary endpoint and key secondary endpoints. This is the first SMA gene therapy clinical trial worldwide to adopt a superiority design, conducting a head-to-head, randomized controlled study against an existing therapeutic drug, and it marks a decisive step for GC101 from clinical research toward market approval. GC101's confirmatory trial meeting its study endpoints represents a key advance in China's SMA drug development field, and it is expected to become the first domestically developed SMA therapy to be approved for marketing.

Underpinning this result is a complete and rigorous chain of evidence:

  1. Multiple Phase I/II studies — conducted across 5qSMA participants of different ages, disease subtypes, and disease stages, these trials have accumulated extensive clinical evidence on the safety and efficacy of the therapy in the broader SMA population.
  2. Phase III pivotal study — enrollment, dosing, and 12-month follow-up have been completed for all subjects, data cleaning has been completed in parallel, and the study has met its primary endpoint.
  3. Designations and recognitions — designated a "Breakthrough Therapy" by the Center for Drug Evaluation (CDE) of the National Medical Products Administration in December 2024; granted Orphan Drug Designation by the U.S. FDA in November 2025, the company's third FDA orphan drug qualification following GC301 and GC310.

Together, studies across all phases have validated that single-dose intrathecal administration of GC101 demonstrates a favorable benefit-risk profile in 5qSMA patients, constituting a complete evidence system from proof of concept to efficacy confirmation.

From the first subject dosed in 2022 to the Phase III primary endpoint achieved today, a domestically pioneered intrathecal 5qSMA gene therapy has completed its pivotal leap.

Disease & Mechanism: The Question of the Disease, the Answer of the Mechanism

A rare disease in captivity, and a single-dose gene therapy

Spinal muscular atrophy (5qSMA) is an autosomal recessive genetic disorder caused by mutations in the SMN1 gene that lead to motor neuron degeneration, resulting in muscle weakness and atrophy, which can be life-threatening in severe cases. The carrier frequency of the pathogenic gene in the Chinese population is approximately 1 in 42, and the neonatal incidence is approximately 1 in 10,000. Although existing treatment options have become available, they are expensive and require long-term medication, placing them beyond the reach of the vast majority of patient families. Affordable, accessible, and full of hope — this is both the question of our times and Genecradle's answer.

GC101 is the first domestically developed AAV gene therapy product for 5qSMA suitable for intrathecal administration. It is delivered as a single intrathecal injection, delivering the gene encoding the normal SMN1 protein directly to the central nervous system to achieve sustained SMN protein expression, thereby improving motor neuron function at the etiological level. Unlike antisense oligonucleotide and small-molecule approaches, which require long-term, repeated dosing, gene therapy aims for one dose with long-term expression.

Platform: Systemic Capability — From a Single Pipeline to a Full Set of Capabilities

The result is supported by systemic capability built on a full-chain core technology platform. Genecradle Therapeutics has established full-chain core technology platforms including a drug discovery and design platform, a vector capsid engineering platform, a genetic element engineering platform, a quality testing technology platform, and a process development and validation platform. The company has built a GMP-grade AAV viral vector production facility, and its R&D and production base of more than ten thousand square meters is now in operation. From target selection and vector design to sequence optimization, preclinical validation, process development, and large-scale production, all critical steps are independently controlled by the Genecradle team; multicenter clinical trials are advanced in collaboration with multiple authoritative medical institutions.

Beyond GC101, the company is simultaneously tackling the industry-wide challenge of pre-existing antibodies: in 2026, the Phase I clinical trial of GC801 injection was initiated, and the first exploratory clinical data from a neonate, using GC801 to lower the pre-existing anti-AAV9 antibody barrier prior to gene therapy, has been published online in preprint form. From the very front line of disease defense to the deeper stages of clinical treatment, and from rare diseases to broader disease areas, the company's footprint is expanding from a single pipeline toward systemic capability.

Horizon: Answering the Question of "Medical Innovation Capability" with Source Innovation

The National Conference on Health and Health Care was held in Beijing on October 12–13, 2026. President Xi Jinping attended the conference and delivered an important speech, noting that the "15th Five-Year Plan" period is a critical period for achieving the goal of building a Healthy China by 2035, and elucidating the capability coordinates for building a Healthy China: it should possess strong capabilities in health management, disease prevention and control, medical services, medical insurance support, medical and pharmaceutical innovation, and talent cultivation.

Of these six capabilities, medical and pharmaceutical innovation is the foundation underpinning all the others. Domestically pioneered, high-quality drugs are the core force fortifying this foundation.

From "keeping pace" to "running alongside" to "taking the lead," every leap forward in China's pharmaceutical innovation must ultimately return to a simple starting point — letting the people access better and new drugs sooner, and letting patients with rare diseases find them affordable, accessible, and full of hope. This is both the question of our times and Genecradle's answer.

Genecradle Therapeutics will continue to dedicate itself deeply to the field of gene therapy, moving forward with greater resolve on the path of source innovation, and contributing to the building of a Healthy China during the "15th Five-Year Plan" period.

The Healthy China we aim to build is one that is balanced, inclusive, and of high quality and efficiency. It should possess strong capabilities in health management, disease prevention and control, medical services, medical insurance support, medical and pharmaceutical innovation, and talent cultivation; it should be able to effectively meet the people's ever-growing health needs, with major health indicators continuing to improve steadily.
